A physician finishes dictating. They dock the recorder. The software encrypts the file, routes it to the server, posts it to the EHR, archives it. The physician didn’t make a security decision. There wasn’t one available.
That was intentional. Every place a user could make a mistake was a compliance risk. So we removed those places. Not with warnings. Not with required fields. Removed.
The market had plenty of companies moving transcription files. None of them had solved the front end — which meant every practice using those tools was one distracted PA away from a HIPAA violation. Not through negligence. Through software that kept asking busy people to make security decisions.
Most healthcare breaches are not sophisticated attacks. They are someone who clicked the wrong thing, skipped the configuration step, or just didn’t know. The technology is rarely the weak point. The interface is.
The fix was never a warning or a prompt. It was never asking in the first place. When the secure path is the natural path, people make fewer mistakes. They do not need to remember a workaround, interpret a policy, or stop to wonder if they handled something correctly. The system handles more of what it should. Security lives in the hierarchy. The boundaries are native.
Do not hand the difficult part back to the customer.
When Anthropic built Claude, they trained the model to evaluate its own behavior against defined safety boundaries — not as a filter added on top, but as part of how it thinks. Deloitte and Accenture didn’t choose them for the feature list. They chose them because their CISOs could say yes without a six-month review. The compliance was already done.
